Julia Child described this history on an episode of The French Chef. She explained that Cardini improvised the dish when a hungry party arrived at closing time and not many ingredients were on hand. She said the entire leaf is to be picked up and eaten from the hand, but didn’t mention how to eat the croutons. The salad in the video seems to have too much dressing to be eaten this way.

Not me, until I saw the Stein programme in November. Assumed it was an American invention.
